Occupational Therapy Services

Full Evaluation

$500.00 - Appropriate for children who are 4 years and older.  Testing of fine/visual motor, gross motor, and sensory motor skills/levels included.  A full written report with explanation of scores, a score sheet, and treatment plan is provided.  Testing/post testing discussions can last up to 2 hours.

Partial Evaluation (Scores Only)

$300.00 - Appropriate for children who are 4 years and older.  Testing of fine/visual motor, gross motor, and sensory motor skills/levels included.  A score sheet and a treatment plan is provided.  Testing/post testing discussions can last up to 2 hours.

Toddler Evaluation

$425.00 - Appropriate for children who are 3 years and younger.  Testing of fine/visual motor, gross motor, and sensory motor skills is included.  A full written report with explanation of scores, a score sheet, and treatment plan is provided.  Test/post testing discussions can last up to 90 minutes.

Handwriting Evaluation

$400.00 - Appropriate for children who are 6-8+ years of age.  Testing of visual motor, sensory motor, and handwriting skills is included.  A write up with explanation of scores and a treatment plan will be provided.  Testing/post testing discussion can last up to 60 minutes.

Treatment Plan

$100.00 - A detailed plan that includes long-term and short-term objectives that highlight the focus of therapy for a specified amount of time. 

Sensory evaluation

$400.00 per hour - Appropriate for children of all ages. This includes fine motor and gross motor testing with emphasis on sensory processing and regulation. A write up with explanation of scores and treatment plan are included.

Therapy Session, clinic

$130.00 per hour - 60 minutes total, breakdown of the session is as follows: 50 minutes of individual, client-centered therapy, working towards achieving goals set forth in the treatment plan; 10 minutes of documentation and wrap-up to help with carry over in the home or school environments. 

Therapy Session, Home or School

$140.00 per hour - 60 minutes total, breakdown of the session is as follows: 50 minutes of individual, client-centered therapy in the clients home or school, working towards achieving goals set forth in the treatment plan: 10 minutes of documentation and wrap-up to help with carry over in the home or school environments. 

Missed or late cancelled Therapy Session

$60.00.  We ask for at least 24 hours notification for a missed appointment.  It should be noted that we understand that children often become ill within this time frame, so if your child wakes up sick on the day of your appointment please let us know no later than 8am.  A call later than that can result in a "late cancel" charge.  We will make exceptions to special circumstances. 

Telehealth Treatment Session

$130 per hour-60 minutes total, breakdown of the session is as follows: 50 minutes of individual, client-centered therapy over a HIPAA compliant virtual platform, working towards achieving goals set forth in the already established treatment plan: 10 minutes of documentation and wrap-up to help with carry over in the home.
